Can anyone refer me to a quality merchant account provider that I can integrate into my site? Quality meaning, one you have used that you've found to be:
* technologicaly reliable
* technologicaly flexible
* low cost (little to no setup / application fees)
* low discount rates (Authorize.net's fees have gotten too high)
* an all-in-one solution that doesn't require bank cooperation
Thanks in advance.

Additionally, I'm not looking for 3rd party off-site solutions like 2Checkout or PayPal as I'll need to store encrypted CC info on a server we control. Thanks.

158 views and no one knows of a good merchant account provider? That's pretty sad if no one has a good enough provider that they feel comfortable reccommending them.

I was going to suggest Authnet.com but I see you must not be happy with them.
I do have to agree that their fees are a little up there.

Yes.
1. Some providers require you to be a resident in their country eg some US providers.
2. Some providers require a bank account to be set up in the country in which the provider is resident eg some US providers
