Is it possible to get good rankings on American search engines even though the site is foreign? The site I am talking about is a .it (Italy) and the content is in German, Italian and English.

I am thinking the search engines rank .com/.net.org etc...ahead of .it/.eu etc...

THanks for your help.

No, they really don't do that. The search engines look at where a site is hosted first and then they consider the language (Italian, German, Spanish, etc.). They then consider the TLD (.co.uk, .it, .eu).

To make it easier, though, you could host a .com site in the USA and make sure it's in English. You'll also need links from other sites, as well.
